free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to
crushing/amputation hazards:
a) The foot treadle control on the Pepto Shear (No. 137g) used to cut
plastic sheets in the Frame Assembly Area was not equipped with a
guard to prevent accidental activation of the machine (point of
operation not guarded), thus exposing employees to amputation type
injuries, January 19, 1988.
