General-duty citation text

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to
an amputation hazard:
a) Employees in the Garage use a Buffalo Unviersal Iron Worker (serial
#46W9008) to shape metal. The machine foot control was not guarded
thus exposing employees to amputation or crushing injuries from
inadvertent activation of the machine, August 25, 1988.
Among other methods, feasible and acceptable abatement methods to
correct this hazard is:
1) Install a barrier guard over the foot control.
2) Install foot control locking device.
3) Install point of operation guarding.
