Employee crushed by falling glass pack
Investigation abstract
Employee #1 was working as a glass processor on the cold end of the main line. A ing the hoist away. The glass pack toppled and struck him. He sustained severe l acerations and crushing-type injuries and died about an hour after the accident. The monorail hoist and load line had been overloaded. The top feet of the glass grabber may have hung up on broken strap brackets on the top frame of the wagon . jumbo glass pack, comprising nine liters of glass, each 128 in. by 0.5 in. and weighing about 1,134 lbs, was positioned on a tilt table. The employee was requi red to pick the pack off the table with a glass grabber (manufactured in house), a device weighing approximately 1,140 pounds. The glass grabber was suspended b y a 5-ton, pendant-controlled, monorail hoist, manufactured by the American Cran e Company. The employee was to use the glass grabber to place the pack onto a la rge wagon. He performed this task without assistance. He operated the grabber an d crane controls to release the glass pack onto the wagon. He then began trolley
