EMPLOYEE STRUCK AND KILLED BY BEAM WHICH FELL FROM TRUCK

Investigation abstract

A BEAM WAS BEING TRANSPORTED BY TRUCK TO A CONSTRUCTION SITE. THE BEAM WAS 97 FE , WELL BELOW THE 26.33 INCH CENTER OF GRAVITY. ET LONG, 54 BY 48 INCHES AT THE TOP, TAPERING TO 54 INCHES BY 38 INCHES AT THE B ASE, AND WEIGHED APPROXIMATELY 38 TONS. THE TRUCK WAS TRAVELLING ON A DIRT ROAD. AFTER MAKING A SHARP LEFT TURN AROUND A HAMMERHEAD COLUMN AND ONTO A BRIDGE, TH E TRUCK DRIVER BEGAN SLOWLY BACKING THE TRUCK. AS HE WAS BACKING, THE BEAM BEGAN TO SHAKE. SUDDENLY, IT ROLLED TOWARD THE SIDE OF THE TRUCK WHERE THE HAMMERHEAD WAS LOCATED. IT ROLLED OFF THE SIDE OF THE TRUCK AND STRUCK EMPLOYEE #1, WHO WA S WALKING THERE. THE BEAM HAD NOT BEEN ADEQUATELY BRACED AGAINST LATERAL MOVEMEN T. THE SADDLE BRACE BEING USED EXTENDED ONLY 12 INCHES FROM THE BASE OF THE BEAM
