REPAIRMAN CRUSHED IN WELDING PRESS
Investigation abstract
EMPLOYEE #1 WAS MAKING REPAIRS ON A LARGE WELDING PRESS. HE POSITIONED HIS UPPER INSIDE THE POINT OF OPERATION WHILE ALL THE POWER SWITCHES WERE IN THE "ON" POS ITION. THE NEW PART HE PLACED IN THE PRESS ACTIVATED A LIMIT SWITCH, WHICH, IN T URN, CYCLED THE PRESS DOWN. EMPLOYEE #1 WAS CRUSHED IN THE PRESS.
