Employees injured by collapse of formwork of bridge span
Investigation abstract
Employees #1, #2, and #3 were working on a bridge span while concrete was being poured. When the formwork support system failed and collapsed, one employee fell 20 ft to the ground and two other employees became entangled in the deformed re bar.
