Employee killed when struck by falling steel beam
Investigation abstract
On November 3, 1986, Employee #1, a welder/ironworker, was preparing to weld a 6 0 ft long steel beam. The 5,500 lb beam was being lifted by a 125-ton P & H cran e. While the beam was being hoisted into place, it slipped in the nylon sling an d fell approximately 20 ft, crushing and killing Employee #1. A coworker who was standing nearby saw the beam falling and managed to avoid it.
