Event description

Employee killed in fall when stepladder overturns

Investigation abstract

At 9:53 a.m. on July 10, 1987, Employee #1 was standing on the eighth rung of a have been a contributing factor. 12 ft stepladder located at the bottom of an elevator shaft. He fell with the st epladder to the concrete floor of the shaft, striking his head just behind the r ight ear on the edge of a hole that had been cut in the floor for a sump pump.
