ON THE EVENING OF JANUARY 29, 1986, EMPLOYEE #1 ENTERED A BATCH TANK. HE WAS OVE RCOME BY A TOXIC AND OXYGEN DEFICIENT ATMOSPHERE AND DIED. IT IS NOT KNOWN WHY T HE EMPLOYEE ENTERED THE TANK.
