EMPLOYEE CRUSHED BETWEEN COUNTERWEIGHT AND MAST
Investigation abstract
Employee #1, along with another employee, was conducting pre-service checks on a r mast and struck Employee #1, crushing him between the counterweight and the ce nter mast frame. dual personnel hoist mounted on each side of a center mast. Both employees foun d the gear drives to be noisy due to the lack of adequate lubrication. Neither e mployee corrected the situation, but both began to use the hoists to lift person nel. During the shift, Employee #1 began to lubricate the gears of one of the ho ists at ground level. He climbed over the hoist enclosure into the center mast a rea. The second employee was called to move his hoist from the 7th floor to the top floor of the building, but did not notice Employee #1 in the mast area.
