EMPLOYEE STRUCK AND KILLED BY PIECE OF FALLING PIPE
Investigation abstract
THREE EMPLOYEES WERE ASSIGNED TO REMOVE A LARGE OVERHEAD PIPE WHICH WAS NO LONGE R NEEDED. THE EMPLOYES CUT OUT A SECTION OF PIPE, 10 FEET, 3 INCHES LONG, WEIGHI NG 348 POUNDS, AND ALLOWED IT TO FALL, UNCONTROLLED, TO THE FLOOR. THE PIPE STRU CK THE FLOOR, BOUNCED AND STRUCK EMPLOYEE #1, WHO WAS WORKING AT FLOOR LEVEL. HE WAS KILLED.
