Investigation abstract
At 4:00 p.m. on May 6, 2021, Employee #1, a 55-year-old worker, was marking trauma injuries to the abdomen. The employee was hospitalized for observation. inventory on stacks of lumber known as packs while working alongside the Green Line Stacker Outfeed Conveyor. The employee was writing pack information onto each advancing stack as it moved along the conveyor, which was controlled by the Stacker operation and conveyor sensors. The packs were 6 feet wide and overhung the conveyor line by 11.5 inches on the side where the employee worked. As the employee walked backward while writing on a pack, he became pinched between the advancing pack and a 34.5-inch-high partial wall near the building's exterior overhead door.
