Event description

Employee sustains fingertip amputation while cleaning print

Investigation abstract

At 2:00 p.m. on June 21, 2021, Employee #1, a 36-year-old full-time pressman, drawing in Employee #1's left hand. He sustained crushing and degloving injuries to the ring finger, resulting in surgical amputation of the fingertip. Other fingers on the left hand suffered minor cuts. Emergency responders transported the employee to a hospital for treatment. The employee was treated without hospitalization. was working alone to address a fisheye print defect on deck No. 8 of Schiavi rotogravure press No. 6 at a commercial printing facility. The press was operating at a normal production speed of approximately 420 feet per minute. While attempting to wipe debris from the left side of the sleeve roller using a job-made hand tool-an 18-inch ink mixing stick with sandpaper, Employee #1 leaned over the doctor blade side of the press and into the area near the unguarded in-going nip point between the impression sleeve roller and the gravure cylinder. The hand tool slipped and was pulled into the nip point,
