Investigation abstract

At 8:45 a.m. on June 28, 2021, Employee #1, a 60-year-old operator, was working amputation of the distal joint of the right index finger, and contusions and lacerations to the right thumb and middle fingers. The employee was treated without hospitalization. at the first pass position on the Double End Tenor (DET) machine in the Door Machining Department. The DET is a semi-automated two-pass door panel sizing system with multiple rotating 11-inch saw blades on each side of the conveyor system. During operation, a door panel stalled at the first saw station due to a piece of wood stuck between the saw blade and conveyor. Employee #1 stopped the machine and then placed both hands on the door edges to remove it. As the door turned, his right hand and fingers contacted the rotating saw blade, which took seven minutes to coast to a stop after power was removed. Employee #1 suffered
