Investigation abstract

At approximately 2:40 a.m. on July 6, 2021, an employee was installing a bladder into a tire curing press using a hoist. The bladder was attached to the hoist via an improper fixture (a J-hook), which became dislodged during the lift. The bladder fell and struck the employee, pinning the employee's leg between the falling bladder and the stripping arm of the curing press. The employee sustained a fractured ankle and was hospitalized for treatment.
