Event description

Employee falls from catwalk and incurs multiple injuries

Investigation abstract

At 9:35 a.m. on July 15, 2021, Employee #1 was on a catwalk approximately 9 feet 5 inches above the floor at the UU1113 section of the flow rack system, selecting product from a pallet on the lower level. Employee #1 placed weight on the pallet, causing it to shift and fall onto a pallet of diaper products about 6.5 feet below. The employee sustained lacerations to the left side of the face and under the eye, as well as fractures to the right arm. Emergency services transported Employee #1 to the hospital, where he was admitted and treated.
