Investigation abstract

At 8:30 a.m. on July 22, 2021, Employee #1 was troubleshooting alignment issues between the pusher and coin discharge on an NM02 vertical coin press. The employee asked a coworker to inch the press forward to observe coin output while his hand was placed on the pusher. The pusher activated, and the employee sustained lacerations to the tips of three fingers. The employee was hospitalized.
