Employee amputates tip of left index finger during maintenan

Investigation abstract

At 8:59 p.m. on May 7, 2021, Employee #1 was performing maintenance on a bread slicer, specifically working on the sprocket and chain assembly. After completing a repair, the employee energized and ran the equipment to validate the correction. While checking the sprocket's teeth for the chain, the employee placed his hand under a feeder belt and it was caught between the moving sprocket and chain. The employee sustained an amputation of the tip of the left index finger. The employee was working alone and was treated in the emergency room at Memorial Hospital-Belleville. The employee was treated without hospitalization.
