Employee is injured in fall through chute door

Investigation abstract

At 1:00 p.m. on August 10, 2021, the plant superintendent was approaching maintenance workers to check on their progress near the press. As he walked across a chute door, it unexpectedly fell open, causing him to fall 16 feet to the lower level of the press onto a steel conveyor. The superintendent sustained a laceration to the head and a possible back injury. He was hospitalized for treatment.
