Event description
Employee?s fingertip is amputated when caught between shovel
Investigation abstract
At 6:00 a.m. on May 24, 2021, Employee #1 was standing on the platform at a galvanizer machine holding onto the metal hand rail while observing the movement of the shovel/ladle used for cleaning the galvanizer. The shovel/ladle struck the middle divider of the holding tank, causing it to move up and down roughly. The employee's hand was struck by the shovel/ladle handle, and the right third phalangeal digit was caught between the metal hand rail and the shovel handle. Employee #1 sustained an amputation of the fingertip and was hospitalized for treatment.
