29 CFR  OSH ACT of 1970 Section (5)(a)(1):  The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees.  a.  On or about May 24, 2021, the employer did not ensure that only designated employees operated the underhung 2 Ton Harrington Hoist, Lot Number ER1A-339, Serial Number 787993, at the Galvanizer #1 machine.  A non-designated, nor qualified employee operated the underhung hoist unsupervised with the wired pendant control.  The employee had repositioned the steel drossing shovel/ladle while conducting the assigned task to clean the galvanizer machine.  As a result, an employee was struck-by the drossing shovel/ladle handle and had their fingertip amputated.
