Employee is killed when struck by oil pipe
Investigation abstract
At 5:30 p.m. on September 28, 2021, an employee was standing on the edge of flatbed trailer while guiding a coworker who was operating a wheel loader to maneuver a heavy oil pipe into place. The coworker was using the forks of the loader to push the pipe when the pipe began to roll towards the employee. The employee was struck in the head by the oil pipe and died due to head trauma.
