Event description

Employee fractures leg when struck by falling steel beam

Investigation abstract

At approximately 7:15 a.m. on October 4, 2021, an employee worked on the CNC feet. None of the beams were hooked up to the overhead crane in the area. No below-the-hook lifting device was attached to the overhead crane's hook. The employees were able to remove one of the beams, which was described as being on his chest, with the beam clamp, and the remaining three beams were removed with a chain sling in a choker configuration. The employee fractured his leg and was hospitalized to treat these injuries. drill machine, which would drill holes in the I-beams before they went to fabrication. Once drilled, the I-beams were stacked three to seven high on railroad beams. The employee would have to move the stack of I beams towards the aisle to make room for the beams coming off the CNC drill machine. The steel beam fell on the employee's leg. There were no witnesses to the incident, but a coworker in the area heard a scream, lifted his welding helmet, and saw the employee with the beams on him. The employee yelled to call 911 and ran over to the injured employee. The beams were 12-26 inches wide and ranged between 20-25
