Investigation abstract

At 9:30 a.m. on October 30, 2021, Employee # 1, a machine operator was feeding wire into the Heller Plate Bending Machine. His hand made contact with in-running rollers, and he was injured. Employee # 1 suffered crushing injuries that led to the surgical amputation of the second and third digits of the right hand. Employee #1 was hospitalized.
