Event description

Employee amputates finger while fixing jammed machine

Investigation abstract

At approximately 7:00 p.m. on November 29, 2021, an employee made a box to pack candy. She was clearing a box of jams on line 17 (stomper). The jam was in the middle of the machine. The employee reached around an interlocked gate/door to clear the jam and the machine activated. She needed to clear a jam. However, she did not activate/press the E-Stop buttons and the dump valve or open the door for the machine to stop running before reaching inside to clear the jam, which resulted in the amputation of her right index finger. The employee was hospitalized to treat these injuries.
