Investigation abstract
At approximately 11:45 a.m. on November 29, 2021, an employee removed a coil and dropped the coil, weighing approximately 1,770 pounds, and landed on the employee's right foot. As a result of the impact, the employee suffered crushing damage to their right big toe, which caused the partial amputation. The employee was hospitalized to treat these injuries. from the coiling machine on Press #6. The employee, a press operator on Press #6, had just finished a run of material on the press. The coiled steel that the employee was using was partially used up during the run and the employee needed to change out the coil stock. The employee moved a 1 Ton Jib crane over to the coiler on Press #6. The employee then used a nylon strap in the basket hitch configuration to lift the coil from the coiler and move it to a pallet on the floor. During the move, the protective sleeve on the sling shifted and allowed a sharpened edge on the coil to slice into the nylon strap. The strap was severed
