29 CFR 1910.184(c)(7): Slings shall be padded or protected from the sharp edges of their loads. On or about November 29, 2021, employees were using a nylon sling to lift a metal coil and did not ensure that the protective sleeve covered the nylon strap which resulted in the strap becoming lacerated and suffering a total load failure.

29 CFR 1910.184(d):Inspections. Each day before being used, the sling and all fastenings and attachments shall be inspected for damage or defects by a competent person designated by the employer. Additional inspections shall be performed during sling use, where service conditions warrant. Damaged or defective slings shall be immediately removed from service. Employees of Brickham Stamping Co, Inc. were not inspecting slings daily to look for signs of defect or damage that would render the sling unsafe for use or structurally unsound.

29 CFR 1910.184(i)(1): Sling identification. Each sling shall be marked or coded to show the rated capacities for each type of hitch and type of synthetic web material. On or about November 29, 2021, a nylon sling used to move metal coils weighing in excess of 1,700 pounds was not equipped with a sling identification tag showing the rated capacity for each type of hitch that sling could be used for.
