Employee amputates fingers while lowering slitter wheel on m

Investigation abstract

At 7:30 a.m. on June 21, 2021, an employee was operating a Bradbury D-coil machine (Model: Coil Car, Serial: 39072) and was in the process of lowering the slitter wheel to cut excess material from a metal coil. During the operation, the employee's left hand came into contact with the slitter wheel, resulting in the amputation of four fingers. The employee was hospitalized for treatment.
