EMPLOYEE DIES AFTER BEING STRUCK BY FALLING DOCK PLATE
Investigation abstract
On January 28, 2022, an employee was clearing a jam on a dock plate leveler. A fter the jam was cleared, the dock plate fell on the employee's forehead. The e mployee was transported to the hospital for treatment of face lacerations and co ncussion. The employee died at the hospital at 8:30 p.m. on March 17, 2022 as a result of his injuries.
