THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a)  ICS Corporation - Variable data center area:  The Chamaleon RF Series Unit attached at the end of the Ricoh Pro VC40000 printing machine had a missing guard  to enclose the machine's moving parts, observed on or about 03/29/2022.  b)  ICS Corporation - Fold Department area: The Speedo 9700 (SP2) Folding machine had a missing guard to enclose the machine moving parts, observed on or about 03/29/2022.  c)  ICS Corporation - Read Write Effects (RWA) area:   The Kirk-Rudy's 497T Servo Attaching system (RWA 6) had a missing guard to enclose pinch points caused by belts and rollers, observed on or about 03/29/2022.  d)  ICS Corporation - Read Write  Effects (RWA) area:  The UV light from the LED curing lamp on the Kirk-Ruby's 497T Servo Attaching system machine (RW5) had a missing anti-glare cover guard to enclose the UV light, observed on or about 03/29/2022.  IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
