Event description
Employee dies of complications following fall from ladder
Investigation abstract
At about 10: 30 a.m. on January 30, 2024, Employee #1, a 69-year-old maintenance behind the knee, respectively). He was diagnosed with NF, prescribed broad spectrum antibiotics intravenously, and his abscesses were incised and drained (I&D). Lab results showed a leukocytosis (high white blood cell count consistent with infection), anemia, azotemia (abnormally high levels of nitrogen-containing compounds in the blood usually related to decreased kidney filtering capacity), hyperglycemia and abnormal coagulation tests consistent with disseminated intravascular coagulation (DIC - a serious condition that causes bleeding, and blood clots in the blood vessels of the body). The employee's aggressively spreading NF required two more I&D of the left lower extremity and left upper extremity, and a left above-the-knee amputation. A mechanic with one year and eight months on the job, injured his left shoulder tissue culture grew Group B streptococcus from the arm tissue; cultures of the leg tissue were negative. The employee developed sepsis (blood infection), which caused difficulty maintaining his blood pressure, and he continued to deteriorate. Following a fourth surgical I&D, the employee's family agreed to palliative care, and the employee died on February 22, 2024, 23 days post injury. and left knee while working from a five rung step ladder. The ladder suddenly moved while the employee was climbing, causing him to use his left hand to stabilize himself and in the process, twist his knee and injure his shoulder. On February 18, 2024, the employee was admitted to the hospital.
