Two employees were attempting to take the forks off of a forklift. One employee was using a pry bar to lift the forks and the other employee was using his hands to push the forks. The pry bar slipped from under the fork, causing it to pinch the fingers of the employee who had been pushing the forks. One of the employee's fingers was amputated to the first joint, while another finger had the very tip pinched off.
