An employee was assisting in installing access plates into a box cover. The chain on the access plate slipped from the narrow portion of the keyhole to the wide portion of the keyhole which caused the plate to be released. The employee had his finger below on the plate frame, and the plate fell onto the employee's thumb amputating it immediately below the nail bed. The plate weighed approximately 1300 - 1400 lbs.
