A road construction crew was using a track-hoe to cover a hole in the roadway with a steel plate. The chain rigging used to suspend the plate slipped or otherwise failed, causing the plate to shift. An employee tried to prevent the plate from falling, which resulted in injuries to the right side of the employee's body, including a broken right pinky finger, lacerations to the right arm, a broken right tibia, and five broken bones in the right foot. The employee underwent surgery and was hospitalized.
