An employee was loading a well with hot oil. The employee had turned valves off and started to fill the well. The pressure in the tank filled due to the hot oil. The pressure release valve released flammable vapors and/or gases. The flammable vapors and/or gases ignited, burning the employee's face (cheeks and ears).
