Final narrative
An employee was operating a hammer mill that kept getting jammed. The employee shut down the machine and opened the access door, which had two hinges on both sides to allow the right or left side to open. The employee had removed/unlocked both sides, which caused the door to fall forward. The employee attempted to catch the door, and his right hand got caught between the metal mill door and a metal support beam. The employee was hospitalized and required surgery.
