Final narrative
An employee was running a brake press and had a piece of rectangular metal. The employee set the dies and pressed (bent) the short end. The employee then turned the piece to bend the long side, adjusted the dies, and leaned forward. The employee's left-hand fingers got caught in the equipment. The employee's left index fingertip and middle finger to the knuckle were surgically amputated; there was also some damage to the ring finger.
