On January 5, 2017, at approximately 10:20 AM, employees were moving a small safe in order to renovate the floor and wall behind the safe. They attempted to lift the safe one corner at a time to place a shim under each corner. While they were placing a shim under a corner, the safe slipped off the pry bar and caught the injured employee's left-hand index finger, crushing it to the middle knuckle. The finger had to be amputated.
