Final narrative
Two employees were removing an air bladder shaft from a paper core inside a plastic roll. The roll was on a pallet on the floor. An employee was stationed at each side of the roll. The injured employee (temporary employee) was pushing the air release valve signaling the other employee to remove the shaft. When the shaft was removed, the injured employee's right index finger was caught between the core and the air release valve causing a fracture/laceration to his right index finger. The injured employee was wearing cut resistant gloves.
An employee was operating a forklift and unloading a truck. After the truck was unloaded, the employee got off the forklift and proceeded to adjust the dock plate with a hook tool. The dock plate slipped and the employee went to adjust the plate with their hand when their left middle finger got caught. Their finger was amputated above the first joint.
