Final narrative
An employee was operating an electric pallet jack and moved it onto the lift gate. The lift gate
was wet and slippery from running a previous pallet that contained product packed in ice. The jack starting sliding
toward the curbside of the platform. As the pallet was sliding to the edge, the associate tried to
keep the pallet level by placing his arm on the pallet, to keep it from falling. The pallet jack continued to slide and as the employee attempted to get out of the way, his finger slid across the pallet jack and was amputated.
An employee parked a short tractor used to move trailers and then exited it. The tractor began rolling and the employee went to jump into the cab to stop the vehicle but his arm struck a metal bar on the cab and he fell, fracturing his left arm. The employee was hospitalized and required surgery.
Two employees were in the basket of an aerial lift. When attempting to bring the basket down, the basket got hung up on a perimeter guardrail post. The basket moved, causing one employee to strike their chest against the lift's guardrail. The employee sustained a fractured rib and was hospitalized.
An employee was working on a pipe-threading machine, placing caps on the end of a pipe. The employee's finger caught the sharp edge of the pipe and was severely lacerated. The employee was hospitalized.
An employee was cleaning a ribbon blender. He had the lid open, thus disengaging the interlock switch and disabling the rotating paddles. After he finished cleaning it, he closed the lid. The blender has a discharge hole at the bottom; unaware that the paddles in the blender were still rotating at a slow rate, the employee reached up through the discharge hole. The paddle amputated the tip of his index finger.
An employee was removing boxes from a conveyor belt when the belt grabbed his gloved hand. The employee pulled his hand back and amputated his fingertip.
