An employee was performing preventive maintenance on a conveyor and was wiping down the conveyor frame with a rag when the rag got caught between the conveyor belt and the roller. The employee's right hand was pulled into the machine and his right middle and index fingers were severely lacerated resulting in amputation. The conveyor was not locked/tagged out at the time.
