An employee was operating a slitter machine which unrolled steel coils. The coils develop slack as they are unrolled and to counter the slackness the operator manually inserts cardboard slats. As the employee inserted a slat, a gloved right hand was pulled into the coil. The middle, ring and little fingers were amputated. The thumb tip was also amputated.
