Final narrative
An employee was chipping gunite from the underside of steel beams using pneumatic tools while in an aerial lift. A large piece of material (approx. 600 pounds) broke loose and fell into the lift basket, overloading it and putting it into an automatic slow descent. The employee fell and the lift basket struck him resulting in multiple cracked vertebrae, and head, wrist and knee injuries.
