Final narrative
On 6/16/2017, at approximately 1100, an employee was pushing a rack up a ramp when a forklift carrying a bin came up behind him. The forklift operator attempted to pass the employee and the side of the bin struck the employee, fracturing his ankle/leg and requiring hospitalization.
