Final narrative
Two employees were un-spooling a wire rope when it became snagged on a flange. When it came loose, it caught one of the employees and spun him around the reel. He hit a (powered off and protected) saw and then the ground, suffering a concussion and bruise on the brain. He was hospitalized.
