On 7/6/2018, an employee was operating a vertical mixer and was using a rag to press against the rotating shaft. The rotating shaft then caught the rag and pulled the employee's right hand into the mixer, resulting in a pinky finger amputation as well as a fracture and abrasions to the right wrist. The mixer was guarded at the time.
