At about 7:20 p.m. on November 13, 2018, a maintenance worker was installing a plastic clevis pin in the fully extended ram of an air-pressurized pneumatic cylinder. The clevis/pin ram caused a fracture and laceration to his right index fingertip that later resulted in a medical amputation.
