On 11/17/2018, at 4:00 AM, an employee was working in an enclosed trailer at a customer's location, preparing down-hole tools using a break wash chemical. He was in close proximity to a space heater that ignited the fumes from the chemical. This caused second degree burns to the employee's face, requiring hospitalization.
