An employee was working with a team to move a forming mold: a large steel tooling that weighed about 4,000 pounds. The employee was pushing the cart that held the mold in position. The mold did not position correctly; it kicked back, pinching the employee's right ring finger against the roller wheel on the cart. The finger was amputated.
