An employee was operating a lathe and was sanding a 1.5 x 20-inch chrome pipe with sand paper when the sand paper became caught on the rotating chuck. The sand paper grabbed his glove and pulled his right hand and arm into the machine, up to his neck. The employee suffered a severe laceration to the face and neck.
